Partial Rotator Cuff Repair With Biceps Rerouting and Double Tenodesis: An Efficient and Cost-Effective Biological Superior Capsular Reconstruction
نویسندگان
چکیده
Background: Chronic rotator cuff tears (RCTs) are common and often only partially repairable. Surgical treatment is challenging in younger patients. options include partial repair, tendon transfer, subacromial spacer, superior capsular reconstruction (SCR), reverse shoulder arthroplasty. The use of SCR has been expanded commercialized. proposed techniques complex using free avascular grafts up to 7 anchors with associated increase theater time nonrefunded cost. Biceps shown promising biomechanical resistance seems offer a simple cost-effective alternative. Indications: Patients RCTs (Goutallier stage ≥3, Patte 3) without arthritis that at least repairable (infraspinatus subscapularis) candidates. mechanically intact long head biceps (LHB) labrum anterior posterior (SLAP) anchor (minimal fraying <10% SLAP full-thickness acceptable) also Technique: Key steps arthroscopic release/lateral opening the bicipital grove (15-20 mm) placement first footprint 8 10 mm anatomical sulcus. Use 5-mm burr create new rerouting groove obliquely from original groove, 15 20 caudal summit tubercle. Lasso-loop translation tenodesis LHB anchor. second lasso-loops end groove. These 2 bipedicle performing function both an tenodesis. Single-row, tension-free over-the-top repair infraspinatus bursal layer supraspinatus completed third on rerouted which remains continuity. Results: pilot series (n = 10) mean follow-up 12 months (9-18 months) shows satisfactory outcomes. One patient developed postoperative frozen one secondary Popeye deformity. Functional scores satisfaction improved all cases. subjective value 30% (10%-40%) preoperatively 75% (60%-80%) postoperatively constant score 30 points (20-40) 68 (60-71). Conclusion: As as its adequate, combination safe alternative time-consuming expensive commercialized techniques.

Arthroscopic Rotator Cuff Repair With Mini-open Subpectoral Biceps Tenodesis
With a range of tear characteristics such as chronicity, degree of fatty atrophy, and number of tendons involved as well as varying patient-specific characteristics including age, injury mechanism, and expectations after treatment to consider, proper and successful treatment of a rotator cuff tear is multifactorial and, consequently, challenging. Although conservative management of a rotator cu...
متن کاملSuperior Capsular Reconstruction With Superimposition of Rotator Cuff Repair for Massive Rotator Cuff Tear
Massive rotator cuff tears are particularly challenging to treat and severely limit the function of the shoulder. Compared with a small rotator cuff tear, massive tears are more unpredictable and usually present with low tendon quality. When performing an anatomical repair of the rotator cuff, the surgical treatment is often associated with failure of the construct. An alternative procedure tha...

BACKGROUND Massive rotator cuff tears in active patients with minimal glenohumeral arthritis remain a particular challenge for the treating surgeon. METHODS A selective literature search was performed and personal surgical experiences are reported. RESULTS For patients with irreparable rotator cuff tears, a reverse shoulder arthroplasty or a tendon transfer are often performed. However, bot...
متن کاملSuperior labral tears: repair versus biceps tenodesis.
Tears of the superior labrum were first described by Andrews et al in 1985 as injuries caused by traction due to throwing. In 1990 Snyder and Karzel published their comprehensive description of the labral/biceps attachment pathology, coining the name ‘‘SLAP (superior labrum anterior-to-posterior)’’ lesion. Since that time, understanding the nuances associated with etiology, diagnosis, and treat...
متن کاملArthroscopic repair of anterosuperior rotator cuff tears combined with open biceps tenodesis.
PURPOSE The purpose of this study was to look at the functional outcomes of arthroscopic repair of anterosuperior rotator cuff tears with open biceps tenodesis when indicated. METHODS We retrospectively reviewed the cases of 17 patients (17 shoulders) who underwent arthroscopic repair of anterosuperior tears with concurrent open biceps tenodesis. At final follow-up, an independent examiner co...
